A therapist can assign a questionnaire in the Minddistrict platform. This allows the therapist to gain insight into the client's mental well-being. These questionnaires can also be offered repeatedly in a schedule to create a measurement pathway.



Create a questionnaire

Creating a questionnaire? This basically works the same as creating a module (topic).



  1. Navigate to the Catalogue page
  2. Click “Create” in the upper right corner



  3. This will bring you to the 'Create content' page



  4. Select the 'Type of content' as “Module or questionnaire” and click “Next” to continue



  5. Give your questionnaire a title, select the “Questionnaire” tool category and your language of choice
  6. Click “Create” to confirm your action and this will bring you to the Topic CMS (content management system
    If you select "-", you will be able to create a module.


  7. You are now able to create the content for a questionnaire in the CMS, as you are used to with creating modules

  8. Your questionnaire is ready when you save your content in the CMS.

To assign the questionnaire to a client, you need to add it in a Questionnaire planning (Topic list).

Questionnaire planning (Topic list)

To make a (series of) questionnaires available to clients, create a questionnaire planning.


  1. Navigate to the Catalogue page
  2. Click “Create” in the upper right corner



  3. This will bring you to the 'Create content' page



  4. Select the 'Type of content' as “topic list” and click “Next” to continue



  5. Give your questionnaire planning a title, select the “Questionnaire planning” tool category and your language of choice
  6. Click “Create” to confirm your action
    Note that if you select the minus sign (-) then you are going to create a module list and not a questionnaire planning.


  7. This till bring you to the 'Preview' page
  8. Navigate to “Template” in the left side menu to add topic content (Modules and/or Questionnaires) in the Questionnaire planning list



  9. Type the titles of the topic content you would like to add to your planning in the catalogue search bar
    You can also use the content ID code.


  10. You will see an overview with all the questionnaires and modules you can add to your planning
  11. When you click “Add” on the questionnaire of choice, it will be added to the planning on the top of your screen



  12. Set an activation date for all your added questionnaires
  13. Press “Save” to confirm the items in your questionnaire planning.
    You are also able to add meta data to the questionnaire planning like you are used to with modules.
